All Students entering 11th grade are required by the state to have a physical examination. I am sending a form home now becasue many students have physicals over summer vacation. I would like to have a physical that is your child's most current so that all immunizations are up to date. The physical form can be returned to me anytime between now, April '08, and the beginning of next school year September '08. If your child has been diagnosed with an allergy to bee stings, insects or any type of food , please make sure you have sent in the appropriate medication such as Benadryl and/or Epi Pen as prescribed by your doctor along with the school medication permission form.

Physical Form
Immunization Requirements:
1.) Varicella (Chicken Pox) immunity, either from vaccination, history of having/had chicken pox or laboratory testing for NEW school enterers (Kindergarten or first grade) and for students entering 7th grade.
2.) 3 doses of Hepatitis B vaccine for students entering 7th grade.
Parents and students are reminded that in order to take any medications during school hours a Dr. note is needed. This applies whether the medication is a prescription drug, or over-the-counter one. Please see Mrs. Curcio for permission form.
*Attention All Parents:
Please be aware: Childhood Immunization against Perussis ( or Whooping Cough) wears off five to ten years after the last routine vaccination shot, typically administered when children are between age 4 and 6. There were almost 20,000 cases in more than 40 years. The first symptoms of Pertussis often mimic the symptoms of an upper respiratory infection including: A low-grade fever, A runny nose, A cough that progressively becomes more severe. Pertussis is spread from airborne droplets when an infected person coughs or sneezes.
A booster vaccine was recently approved by the U.S. Food and Drug Administration and is available in most Doctors offices. Please contact your family doctor concerning this information.
Many students are coming into the Nurses office requesting over the counter medications (for headaches, cramps, etc) but do NOT have a Doctors note on file and do NOT have their own supply of medication here. No one will be given any form of medication without proper authorization from your Doctor. The first time I will contact a parent for verbal permission and the proper form will be sent home with the student at that time for your Doctor to complete. After this nothing will be given unless the form is returned signed by your Doctor.
