A reminder to all 11th grade students:
There are still many of you who have not turned in the required 11th grade physical form to Mrs. Curcio. Please submit them ASAP. Any questions please see Mrs. Curcio.
Please be Advised:
All Students entering 11th grade are required by the state to have a physical examination. I am sending a form home now because many students have physicals over summer vacation. I would like to have a physical that is your child's most current so that all immunizations are up to date. The physical form can be returned to me anytime between now, April '09, and the beginning of next school year September '09. If your child has been diagnosed with an allergy to bee stings, insects or any type of food , please make sure you have sent in the appropriate medication such as Benadryl and/or Epi Pen as prescribed by your doctor along with the school medication permission form.

Medications During School Hours
Parents and students are reminded that in order to take any medications during school hours a Dr. note is needed. This applies whether the medication is a prescription drug, or over-the-counter one. Please see Mrs. Curcio for medication form. or access it online.Medication-Refer to page 6 in student handbook for further information.
All students who take prescription or over the counter medications during school hours need to have a medication form completed, signed by your family doctor, and turned into Mrs. Curcio in the nurses office. We ask that you also bring in a bottle of the medications labeled with your name to be kept in the nurses office for you to access when needed.
